Initial diagnostic consultation
(90 minutes)
$475
Includes review of records, psychiatric assessment, diagnostic impressions, and treatment recommendations, this can include psychotherapy and/or medication recommendations.
Meeting once with Dr. Lemkin does not form an immediate doctor- patient relationship.
After consultation, Dr. Lemkin with make a decision if it is appropriate to establish a therapeutic relationship. If it is decided that a patient requires treatment with another psychiatrist, Dr. Lemkin will facilitate the appropriate referral.
Follow Up
(30 or 60 minutes)
$150 or $300
Follow up appointments for established patients include therapy and/or medication management. Scheduling of follow up appointments vary and are tailored to each patient.
One Time Consultation
(90 minutes)
$475
Dr. Lemkin sees patients in treatment for an additional opinion. Follow up consultation session fee is the same.

Dr. Lemkin does not provide disability evaluations or sign disability claims as this represents a conflict of interest.
Dr. Lemkin will not testify on your behalf or provide copies of records or reports unless legally obligated to do so by a judge.
Cancellations
Appointment times are reserved for each patient. 24 hours notice is required to cancel this reserved time. If a patient cancels less than 24 hours in advance, the full cost of the session will be charged to your payment method on file. This excludes any medical emergencies.
Insurance
Dr. Lemkin is an out-of-network provider for all insurance carriers. She does not accept any insurance. Payment in full will be charged after each appointment.
Dr. Lemkin cannot see patients with Medicare, however, regardless due to policy.
Each patient will be given a receipt for services provided to submit to an insurance company with all the necessary information. Insurance that includes out-of-network benefits have reimbursements that cover only a portion of the out of pocket expenses. Often insurance reimburses 20-90% of fees.

Dr. Lemkin is listed under various insurance panels due to past and current hospital and work affiliations.
However, this private practice is a separate entity.
Unfortunately, the current healthcare system and insurance companies create difficulty for providing the best mental health care. Consequently, Dr. Lemkin does not participate in insurance networks or bill carriers directly.
In checking with your insurance carrier to see the reimbursement rate, it may be helpful to report the codes that will be billed. You will need to know if you have out-of-network benefits, any deductible for out-of-network benefits you must meet, and the dollar reimbursement amount for the following codes:
